He was introduced as the Killer of CHAPTER IV: Spark of Madness, a Chapter DLC released on 11 May 2017.
Overview
The Doctor is a madness-inducing Killer, able to use his Power - Static Blast, incapacitating Survivors and causing them to hallucinate and scream in terror, revealing their position.
His personal Perks, Template:IconLink, Template:IconLink, and Template:IconLink, encourage chases, impact Survivors' ability to repair effectively and make him harder to detect when not in a Chase.
His Perks are about hindering and creating problems for the Survivors. The sheer stature of The Doctor infuses fear and terror in those around him.

From an early age, Herman Carter understood the human psyche. To analyse and de-construct something as powerful as the brain intrigued him. He was an apt pupil and gained the attention of his teachers. He excelled in high-school and was published in "Partisan" — a psychology gazette. Within a year, Carter was fast-tracked into Yale's advanced neuroscience programme, really a front for the CIA. Brainpower is a must if you're about to conquer the world and demolish foes across the pond. The CIA understood this, so interrogation and intelligence became their number one priority. All they needed were brilliant people — like Carter.
Carter and other top-tier recruits were transferred off-campus and into a secret black site facility in Illinois known as Léry's Memorial Institute. A protégé craves a mentor, and that's where Dr Otto Stamper stepped in, who taught Carter that information is everything and knowledge is power. He was given all instruments needed, a guiding hand, and more or less everything he asked for. He never realised that sunlight had started to become so scarce, that he too was kept in the dark. Because knowledge doesn't only give you power, it also transforms you into a threat.
To extract information was his mission. Dr Stamper encouraged Carter to go further and not to consider this a normal medical facility — no eyes were watching them, there were no rules to abide. The agency just pointed Carter in the right direction, then he started to take a few steps back as he saw how Carter could walk on his own. Docile test subjects were exchanged for real, live spies. People that played a role in the troubles outside the facility. Carter shouldered this new role — Project Awakening took form, and on paper Carter described it as "experimental interrogation". It was approved and over a few months, nobody knocked on his door. Screams and moans filled the corridor outside his lab, but wars skew people and what they accept — as long as the enemy is kept at bay. The fluorescent lights flickered more and more often. Electro-convulsive Treatment became a standard dish on the menu. Prisoners held at the facility begged the guards to take them to any other lab but Carter's. Rumours were disregarded in the beginning.
Over the years, Carter became known as The Doctor and no one ever questioned if he had even held a medical certificate or even what happened to the prisoners after they had given up their information. It was only after the Léry's Memorial Institute went silent for a week that they finally uncovered the true horror of what had happened there.
Carter's experimental information extraction had turned to horrific and bizarre torture. Patients and prisoners were found dead or in vegetative states with all types of head trauma. In his office, they found the most terrible discovery of all. Dr Stamper himself, his head peeled open and an array of electrodes and sensors inserted into his still working, but annihilated brain. There was no sign of Herman "The Doctor" Carter, but his research papers suggested that he had been using the prisoners as part of awful ECT experiments as he searched for the panacea of mind control.
The government didn't want to know. The black site was condemned and all knowledge of the Léry's Memorial Institute redacted forever.

Showing exceptional aptitude in psychology, Herman was handpicked for special training in an advanced neuroscience program located in a secret black site facility in Illinois known as Léry's Memorial Institute, really a front for the CIA.
This is where Herman met Dr Otto Stamper, where under his guidance, Herman began to use strange and ever increasingly vicious methods to extract information from prisoners sent to the institute, which was also a covert prison and re-education cenre for whoever was the USA's current nemesis. His liberal use of violent electroshock therapy revealed dramatic results and several threats to national security were uncovered.
Over the years, Herman became known as The Doctor and no one ever questioned if he had even held a medical certificate or even what happened to the prisoners after they had given up their information. It was only after Léry's Memorial Institute went silent for a week that the true horror was finally discovered. The personnel, patients, and prisoners were all found dead with all types of head trauma. All personnel and prisoners' bodies were accounted for, including Dr Stamper, but no sign of Herman “The Doctor” Carter.

- The Static Blast attack range is directly tied to The Doctor's Template:IconLink and can be increased and decreased accordingly.
- The Static Blast attack has a default charge time of 2 seconds.
- The Doctor is slowed to 3.08 m/s while charging the Shock Therapy attack.
- The Shock Therapy attack has a so-called Detonation Delay of 1 second in between spawning the visual effects of the shock wave propagating outwards from The Doctor and it shocking any Survivors within its Area of Effect.

Successfully striking Survivors with The Doctor's special abilities, Shock Therapy and Static Field, increases the Survivors' Madness, eventually afflicting them with the Madness Status Effect and triggering increasingly potent effects:

Static Field Statistics
- Madness Tier thresholds were at 5, 55, and 105 points respectively.
- Madness was passively gained whenever Survivors were within The Doctor's Terror Radius and he was in Treatment mode, indicated by spark VFX at the Survivors' feet.
- The closer they were to a Treatment mode Doctor, the faster the passive Madness-gain.
- Madness passively decayed at a rate of -0.5 points per second when outside of The Doctor's Terror Radius.

- The Doctor introduced Template:IconLink, a new Game Mechanic/Status Effect unique to him.
- The Doctor was the first Killer to have had two different stances, Punishment Mode for regular attacks and Treatment Mode to use his Power.
- Treatment Mode allowed The Doctor to perform grabs, but prohibited any attacks with his primary weapon.
- Since so-called Treatment grabs were virtually guaranteed to succeed, it made it extremely dangerous to go for an unhook against a Treatment mode Doctor in attack range.
- Treatment Mode allowed The Doctor to perform grabs, but prohibited any attacks with his primary weapon.
- The Doctor's Memento Mori animation was the first one in which Survivors saw part of the animation from the Killer's perspective.
- Due to technical limitations, Illusionary Doctors originally used to wear the default customisation option of The Doctor, making them easily distinguishable from Doctors wearing either the Prestige or Project Awakening outfits.

Lore Trivia
- The Doctor's' facial equipment is a reference to the 1971 Movie A Clockwork Orange where the protagonist undergoes brutal psychotherapy treatment in an attempt to cure his mania and bloodlust.
- The Doctor's' background story may be a reference to MKULTRA, a CIA research project that officially ran from 1951 to 1973 and experimented with unethical methods of interrogation and mind control.
- "Herman" was a widely popular name in the US in the 19th century and remained so throughout the first half of the 20th.
- "Carter" is a surname common among African Americans capable of tracing their roots back to the southern United States or Caribbean from the early 20th century onward.
- Some file names reveal that during development, The Doctor was originally called "Mu Yi", a name that sees The Doctor's likely original nationality as either Chinese or Korean.
- The terms "Mu" (無 in Japanese; 무 in Korean) or "Wú" (无 in Chinese) mean "not have; without" and are a key word in Buddhism, especially in Zen traditions.
- The term "Yi" (義 in Confucianism) means "justice, righteousness, meaning", a moral disposition to do good.
- When combined, Carter's original name literally translates to either "without justice" or "without meaning".
- Other file and WIP names of his Add-ons ("Jade Charm", "Nanmu Catapult", "Tiger Fang Charm") also reveal that The Doctor was generally supposed to be more Chinese-themed at an early point in development.
