Survivors have three Health States in the game: healthy, injured and dying.

Health states can affect a Survivor's prowess at surviving and also inhibit certain interactions.

Healthy

Survivors start the trial in this state, with the exception of those using the Perk noMither No Mither}}.

Receiving damage from a Killer's Weapon when healthy puts Survivors into the injured state.

Receiving double damage from a Killer's Power, or a Killer's weapon while healthy puts Survivors into the dying state immediately.

  • The former is the case for The Hillbilly's hillbilly Chainsaw}} and The Cannibal's bubbasChainsaw Bubba's Chainsaw}} and The Shape's shape Kitchen Knife}} when on Evil Within III,.
  • The former is also the case for the Huntress' huntress Hunting Hatchets}} with the iridescentHead Iridescent Head}} Add-on equipped and the Trapper's trapper Bear Traps}} with the diamondStone Diamond Stone}} Add-on equipped.
  • The latter is the case for any Killer using an activated noOneEscapesDeath Hex: No One Escapes Death}} on Tier III.
  • The latter is also the case for any Killer using an activated devourHope Hex: Devour Hope}} on Tiers II/III, with the Killer having received 3 Tokens.

Injured

Survivors using the Perk No Mither start the trial in this health state and cannot be healed to healthy.

While injured, Survivors will bleed and grunt in pain, making them easier to track. Though an injured Survivors limps, their movement speed is unaffected and the same as when healthy.

Receiving damage from a Killer's Weapon while injured puts Survivors into the dying state.

Survivors can reduce or stop the grunting by using either of the Perks noMither No Mither}}or ironWill Iron Will}}:

  • No Mither II reduces grunts by 25% at any time, even in dying state.
  • No Mither III reduces grunt by 50% at any time, even in dying state.
  • Iron Will I reduces grunts by 50% while standing still.
  • Iron Will II reduces grunts by 100% while standing still.
  • Iron Will III reduces grunts by 100% at any time.
  • Survivors can also reduce grunting noises by 50% when crouching still and not moving.

Dying

While in the dying state, a Survivor's movement is limited to a slow crawl. A crawling Survivor has two options: recover as much health as they can, or crawl to find help. Without the aid of certain perks Perks}}, a Survivor cannot recover fully; they will need another Survivor to finish healing them. A crawling Survivor can be picked by the Killer and brought to a sacrificial hook Hooks}}.

While in the dying state, Survivors slowly bleed out till death. The bleed-out progress can be seen below the player's Status Icon. Dying Survivors can see the auras Auras}} of their team mates, and vice versa. Dying Survivors can use their team mates' locations to determine whether they should move or attempt to recover.

  • It takes 240 seconds (4 minutes) for a dying Survivor to bleed out and die.
  • A Killer can see the Aura of a dying Survivor if they have the Perk deerstalker Deerstalker}}.
  • unbreakable Unbreakable}} and noMither No Mither}} allow a Survivor to recover fully from dying state.
  • A Killer using the Perk knockOut Knock Out}} will prevent the other Survivors from seeing the dying Survivor's Aura outside of a certain range, similar to a reverse Deerstalker.
  • Survivors can reduce the grunting by using the Perk noMither No Mither}}:
    • No Mither II reduces grunts by 25% at any time, even in dying state.
    • No Mither III reduces grunt by 50% at any time, even in dying state.

Healing

Healing can put Survivors back into higher health states.

  • It is possible to heal oneself with a firstAidKit Med-Kit}} or selfCare Self-Care}} or being healed by other Survivors back to healthy indefinitely.
    • This is not true for when the injured Survivor is running No Mither, they cannot be healed past injured state.
  • A Killer can see the Auras of a Survivor being healed if they have the Perk aNursesCalling A Nurse's Calling}}.
    • They can also see the Survivor who is healing the other Survivor with Tier III of the Perk.
